Spitex Coordinator / Nursing Specialist HF/FH (60-100%)

On direct behalf of our client, a sympathetic, renowned and rapidly growing Spitex organization (PA-model) with a sympathetic corporate culture, we are looking for an experienced Dipl. Nursing Specialist HF/FH for coordination and case management (without nursing activities) to expand the team. You will strengthen (selectable) the team in the city of Bern or in the city of Zurich (place of work selectable, both central locations, parking available). Spitex Coordinator / Nursing Specialist HF/FH (60-100%) # In this versatile role, you are part of a collegial, dynamic team and, as a competent specialist, you take over case management as well as care planning for the care including preventive support of clients at home. You plan the care including needs assessment, target specifications and deployment planning for the FaGes and caring relatives (PA), who care for the patients on site. You take over case management and coordination with the health insurance companies, stand by the teams in an advisory capacity and support in advising clients (by telephone, occasionally also on site - e.g., during status assessments). Together with your team, you ensure optimal provision of nursing services and client care. As a communicative personality, you also find active cooperation and the exchange of information with various contact persons (management, FaGes, care team, external bodies such as health insurance companies or authorities) easy. Nursing Specialist HF/FH or an equivalent further education (or are in training for it). In addition, you have already been able to gather versatile practice in case management and coordination in the nursing sector (ideally including Spitex). You enjoy communicating with a wide variety of contact persons, accompanying nursing cases with your teams and building a professional relationship based on trust. You work with a high degree of independence, are well organized and have great capacity for introduction as well as a healthy dose of humor.
