(Exclusive) Spitex Coordinator / Nursing Professional HF/FH (60-100%) / Permanent Introduction On direct behalf of our client, a sympathetic, renowned, and strongly growing Spitex organization (PA model) with a sympathetic corporate culture (headquarters in, among others, Stadt Bern and Zürich, central location, parking spaces in front of the building), we are looking for an experienced Dipl. Nursing Professional HF/FH for the coordination of the team and case management to expand the team at the location in Zürich. Nature of Work In this versatile function, you are part of a collegial, dynamic team and, as a competent specialist, you take over case management as well as care planning for the care including preventive support of clients at home. You plan the care including needs assessment, target specifications, and deployment planning for the FaGes and caring relatives (PA), who care for the patients on site. You take over case management and coordination with the health insurance companies, provide advisory support to the teams, and assist in advising clients (by telephone, occasionally also on site - e.g., during assessments). Together with your team, you ensure optimal provision of nursing services and client care. As a communicative personality, you also find the active cooperation and exchange of information with various contact persons (management, FaGes, care team, external bodies such as health insurance companies or authorities) easy. Nursing Professional HF/FH or an equivalent further education (or are currently in training for this). Additionally, you have already been able to gain versatile practical experience in case management and coordination in the nursing sector (ideally including Spitex). You enjoy communicating with a wide variety of contact persons, accompanying nursing cases with your teams, and building a professional relationship based on trust. You work with a high degree of independence, are well-organized, and possess great capacity for introduction as well as a healthy dose of humor.
