(Exclusive) Spitex Coordinator / Nursing Specialist HF/FH (60-100%) / Permanent Introduction Directly on behalf of our client, a sympathetic, renowned and strongly growing Spitex organization (PA-model) with a sympathetic corporate culture (headquarters in, among others, Stadt Bern and Zürich, central location, parking spaces in front of the building), we are looking for an experienced Dipl. Nursing Specialist HF/FH for the expansion of the team at the location in Zürich for the coordination of the team and case management. Type of Work In this versatile function, you are part of a collegial, dynamic team and, as a competent specialist, you take over case management as well as care planning for the care including preventive support of clients at home. You plan the care including needs assessment, target specifications and deployment planning for the FaGes and caring relatives (PA), who care for the patients on site. You take over case management and coordination with the health insurance companies, provide advisory support to the teams and assist in advising the clients (by telephone, occasionally also on site - e.g., during assessments). Together with your team, you ensure the optimal provision of nursing services and client care. As a communicative personality, you also find active cooperation and the exchange of information with a wide variety of contact persons (management, FaGes, care team, external bodies such as health insurance companies or authorities) easy. You enjoy communicating with a wide variety of contact persons, accompanying nursing cases with your teams and building a professional relationship based on trust. You work with a high degree of independence, are well organized and have a great ability to introduce/onboard as well as a healthy dose of humor.
