(Exclusively) Coordinator Spitex / Nursing Specialist HF/FH (60-100%) / Permanent Introduction On behalf of our client, a sympathetic, renowned, and strongly growing Spitex organization (PA model) with a sympathetic company culture (headquartered in Bern and Zurich, central location, parking spaces in front of the house), we are looking for an experienced Dipl. Nursing Specialist HF/FH for team coordination and case management at the Zurich location. Type of work In this versatile role, you are part of a collegial, dynamic team and, as a competent specialist, take over case management and nursing planning for care, including preventive support for clients at home. You plan nursing care, including needs assessment, target specifications, and deployment planning for FaGes and caring relatives (PA), who care for patients on site. You take over case management and coordination with health insurance companies, advise the teams, and support client counseling (by phone, occasionally also on site - e.g., for site determinations). Together with your team, you ensure optimal provision of nursing services and client care. As a communicative person, you also find it easy to cooperate actively and exchange information with various contact persons (management, FaGes, care team, external offices such as health insurance companies or authorities).
