WeHo Spitex GmbH is a specialized outpatient Spitex operation with a focus on psychiatric nursing and the field of carer support (somatic and psychiatric).  We accompany people in their home environment individually, resource-oriented, and at eye level. In the area of carer support, we employ caring relatives who care for their close relatives in the somatic or psychiatric field. As a Spitex, we ensure professional accompaniment, needs assessment, care planning, quality assurance, and documentation control. For the further expansion and professional development of this area, we are looking for a committed, independent, and strong-communicating personality as soon as possible or by arrangement. Nursing Specialist Psychiatry HF/FH (60-80 %) Case Management in Carer Support Who we are *WeHo Spitex GmbH is a specialized outpatient Spitex operation with focuses on psychiatric nursing and carer support.* *We accompany people with mental, somatic, or combined care needs in their home environment - individually, resource-oriented, and at eye level.* Your role In this function, you are the central contact person for clients, caring relatives, as well as internal and external specialists. You bear responsibility for the entire care process - from anamnesis and needs assessment through planning and coordination of care to the regular evaluation of progress. In doing so, you ensure high-quality, traceable, and KLV-compliant service delivery. At the same time, you support and accompany caring relatives in their demanding role. You convey knowledge, provide security in daily care, and contribute to ensuring that care remains sustainable at home in the long term. Furthermore, you contribute your specialist knowledge to the further development of the offering, the processes, and the quality standards. Special characteristic of carer support Carer support operates at the interface of professional nursing, familial relationships, and health policy issues. It requires nursing specialists who can differentiate between nursing, care, and familial support. For this, we are looking for a personality who supports this attitude with professionalism, heart, and backbone. Your tasks Client-related tasks • Needs assessments for somatic, psychiatric, or combined care needs • Case management, progress controls, as well as creation and evaluation of care plans • Home visits and quality controls in the home environment • Telephone and personal assessments with clients, doctors, therapists, and health insurers • Professional representation of service questions towards payers Tasks in the area of carer support • Instruction, training, and accompaniment of caring reference persons • Promotion of operational certainty and resource orientation • Support in role clarification and stress management • Checking of documentation and service records for quality, traceability, and KLV compliance Professional development and quality management • Participation in the further development of internal processes and quality standards • Participation in concepts and professional projects What we are looking for • Completed training as a Dipl.
