Case Management Psychiatry and Somatics (if interested)

Your tasks * You assume case management responsibility for clients with mental and somatic illnesses in an outpatient setting and are the specialist responsible for psychiatric clients. * You conduct structured needs assessments (RAI-CMH/RAI HC) and create nursing plans. * You assess mental states, recognize changes early, and manage appropriate nursing interventions. * You train, coach, and support employees as well as caring relatives in psychiatric and somatic issues. * You ensure that nursing services are provided professionally, documented, and can be billed according to KLV. * You carry out regular quality controls and ensure compliance with professional standards. * You coordinate cooperation with family doctors, psychiatrists, authorities, social services, and health insurance companies. * Through your professional attitude, you contribute to stability, security, and therapeutically effective care in the home environment. Your profile * Registered nurse HF or FH with a focus on psychiatry or at least 2 years of psychiatric professional experience * Experience in outpatient psychiatric nursing * Enjoyment of guidance, coaching, and professional support * Confidence in assessing complex psychiatric situations * High advisory, communication, and social competence * Independent, structured, and responsible way of working * Very good German skills in speaking and writing * Knowledge of RAI-CMH/RAI HC as well as Perigon is an advantage.
