Pflegefachperson with case management, you take responsibility for the nursing process, coordinate the care of our customers, and contribute to their quality of life every day with your expertise. This allows you to stay professionally up to date and continuously expand your knowledge. Your working hours are generally from 7:00 to 16:00. Pflegefachfrau/-mann mit Fallführung - Somatik 80-100% (a) YOUR TASKS * You support our customers in being able to live self-deterministically in their homes for as long as possible. * You take over case management and are responsible for the planning, control, and evaluation of the nursing process. * You advise and accompany customers as well as relatives and create trust through your professional competence. * You coordinate cooperation with doctors, specialist agencies, and other stakeholders and ensure optimal care. * You share your knowledge within the team and accompany new employees, apprentices, and students. * You ensure professional and seamless nursing documentation.
