Certified Nurse with Case Management - Dementia 60-100% (a) Do you want to support people with dementia to live safely and independently in their familiar home for as long as possible? In the Memory Care Team of Spitex Zürich, you will provide holistic support to clients with cognitive impairments - from needs assessment and nursing care to advising relatives. In doing so, you will take on case responsibility and design individual solutions that make everyday life easier for those affected and their environment. Your shifts take place during the day - usually between 07:00 and 16:00. Certified Nurse with Case Management - Dementia 60-100% (a) YOUR TASKS You will take on case management for people with dementia and manage the nursing process from needs assessment and planning to the regular evaluation of the agreed measures. You will nurse and care for our clients in their familiar homes, promoting their existing resources and supporting them in maintaining their independence for as long as possible. You will advise and support relatives, coordinate cooperation with doctors and specialist centers, and act as an important contact person for all parties involved. You will work closely with colleagues from various professional groups within the framework of the Skill & Grade Mix and ensure careful and complete nursing documentation. You will network with internal and external specialists, contribute your knowledge to the team, and support new employees, apprentices, and students during their induction.
