Do you want to support people with dementia in living safely and autonomously in their familiar home for as long as possible? In the Memory Care Team of Spitex Zürich, you will provide holistic support to customers with cognitive impairments - from needs assessment to nursing care and advice for relatives. In doing so, you will take on case responsibility and design individual solutions that make everyday life easier for those affected and their environment. Your shifts take place during the day - usually between 07:00 and 16:00. Dipl. Pflegefachfrau/-mann mit Fallführung - Demenz 60-100% (a) YOUR TASKS * You will take over case management for people with dementia and manage the nursing process from needs assessment and planning to the regular evaluation of the agreed measures. * You will care for and support our customers in their familiar home, promoting their existing resources and supporting them in maintaining their independence for as long as possible. * You will advise and support relatives, coordinate cooperation with doctors and specialist centers, and be an important contact person for all those involved. * You will work closely with colleagues from various professional groups within the framework of the Skill & Grade Mix and ensure careful and seamless nursing documentation. * You will network with internal and external specialists, contribute your knowledge to the team, and support new employees, apprentices, and students during their induction.
