We are looking for Dipl. Fachpersonen HF/FH with case management - Acute and Chroin Care 60-90% for one of our Spitex customers.

YOUR TASKS:

• Resource- and goal-oriented nursing and care
• Advice and support for relatives
• Case management through planning, controlling, and evaluation of the nursing process
• Instruction, coaching, and advice for nursing staff of other competency levels
• Responsibility for careful and complete maintenance of nursing documentation
• Introduction and support of new employees, apprentices, and students
• Interdisciplinary cooperation with employees, specialist agencies, and doctors

REQUIREMENTS:

• Completed nursing training at tertiary level
• Friendly and appreciative appearance as well as physical and mental resilience
• Enjoyment of change and interprofessional cooperation in the team
• High sense of responsibility, independence, and flexibility
• Good knowledge of the German language in speaking and writing, at least level B2
• Driver's license Kat. B or willingness to travel by electric bicycle
